Select one of the following methods for submitting an application for employment to Medical West and follow the instructions listed below.
Fill out Application online & submit via e-mail:
1) Click the *Employee Application* link below. The application form will open in your browser.
2) Fill out the application completely.
3) When you have completed the form, save it to your desktop.
5) Finally, attach the saved Employment Application file to an e-mail and send it to Personnel@uabmw.org You may also attach a resume to this same e-mail if you wish.
6) If you experience any problems with the form, please e-mail our webmaster.

Mail Application In:
1) Click the *Employee Application* link below and the Application document will open in your browser.
2) Fill out the application completely.
3) Print the completed application and sign.
